The three mistakes are

1. the spelling of the English word "consumers"
2. the repetition of "de" in Spanish
3. the word "las" which I think should be "lo" agreeing with "libro"

Besides this there are all sorts of other comments one might make, e.g. why notices often have a heading stating that they are notices, as if they'd otherwise be taken to be bottles of wine or small dogs. One wonders also about the possible implications of the "official" status of the complaint form.
